Grant Application Guidelines for the Foundation Fund

(Application should be no more than 3 pages in length)

1. Name of applying organization with address/telephone/E-mail (if applicable).

In addition, name, contact information and position in the organization of the person submitting the application.

2. Total amount of funds requested.

3. Description of the purpose/project for which the funds will be used.

4. Identify how this purpose/project meets the criteria of the Foundation Fund.

5. Please list details of how the funds will be spent.

6. Please indicate other financial support being sought for this project.

7. Please describe total funding plan for the project, indicating what funding is committed vs. what is hoped for.

8. Does the congregation have charitable status?

PLEASE NOTE: WE REQUEST A REPORT WITHIN ONE YEAR INDICATING THE USE OF THE FUNDS AND HOW THE PROJECT HAD AN IMPACT ON GROWTH.
